Terms of use

Overview
These Terms govern your use of Elivara Health's website, eligibility assessment, checkout, support channels and related services, operated by Elivara Health. By using the website you agree to these Terms and the policies linked from them.
Healthcare relationship
Elivara Health is a technology platform, not a medical provider, and it is not a pharmacy. Medical services are provided by LocumTele, whose licensed nurse practitioners and physician assistants review assessments and write prescriptions. LocumTele is responsible for clinical care, provider licensure and oversight, patient records and clinical policies. Elivara Health provides the website and non-clinical support, and cannot influence a clinical outcome. A provider-patient relationship begins only when a licensed provider reviews your information and establishes it. Website content, the assessment, an automated message or a support conversation does not by itself create a provider-patient relationship or guarantee treatment.
No emergency care
Elivara Health is not an emergency service. Call 911 or go to the nearest emergency room for an emergency.
Eligibility and accurate information
You must be 18 or older and located in a state we serve. We serve 47 states + DC. We do not currently serve patients in Alaska, Mississippi or New Jersey, U.S. territories, or APO/FPO addresses. See State availability. You agree to give accurate, current and complete information and to update it when it changes.
Provider decisions
A licensed provider decides whether an evaluation, treatment, prescription or follow-up is appropriate. We do not guarantee eligibility, a prescription, a specific product, a particular result or a specific review time.
Pharmacy and medication
Prescriptions are filled after a licensed provider issues a valid prescription. Compounded medications are prepared and dispensed by VSDH, a state-licensed compounding pharmacy. Compounded medications are not FDA-approved, and the FDA does not review them for safety, effectiveness or quality. Follow the pharmacy label and your provider's instructions. Never share prescription medication.
Payments, renewals and cancellations
You're only charged if a licensed provider reviews your assessment and approves treatment, usually within 24–48 hours. If treatment isn't approved, you aren't charged and nothing ships. Plans are billed monthly and renew automatically at the price shown before you order until you cancel or your plan length ends. Cancel anytime before your next renewal by emailing support@elivarahealth.com. Refunds are covered in our Cancellation, refunds & shipping policy.
Acceptable use
You may not use the website unlawfully, submit false information, access another person's account, interfere with the service or copy content without permission.
Governing law
These Terms are governed by the laws of the State of Wyoming, without regard to conflict-of-law rules.
